Going to the vet can sometimes feel overwhelming. Did you remember to mention that weird cough? What about the new treat you’ve been giving? This guide and accompanying worksheet are designed to help you prepare thoroughly, ensuring you get all your questions answered and provide your vet with the most accurate picture of your pet’s health. Make every appointment effective and stress-free!


Section 1: Pre-Appointment Checklist (Before You Leave Home)

What to Gather:

  • Current Medications & Supplements:
    • Bring all prescription medications (even if they’re nearly empty) and any over-the-counter supplements your pet is currently taking. A clear way to organize these is with a pet pill organizer, which also helps you remember dosages.
    • Tip: Take photos of the labels if you can’t bring the bottles.
  • Medical Records (if new vet): If this is your first visit to a new clinic, bring previous records or have them sent ahead of time. Keep all your pet’s important documents organized in a dedicated pet medical records folder.
  • Stool/Urine Sample (if requested): Ask the clinic if they need a fresh sample.
  • Your Pet’s Favorite Toy/Treats: Can help distract and comfort your pet.
  • Leash/Carrier: Ensure your pet is safely contained for the journey. For senior dogs or those with mobility issues, a pet car ramp can make getting into the car much easier and safer.

What to Track & Note Down:

  • Specific Symptoms: When did you first notice the issue? How often does it happen? Has it gotten better or worse?
  • Changes in Appetite/Thirst: Is your pet eating or drinking more or less?
  • Changes in Urination/Defecation: Any changes in frequency, consistency, or color?
  • Behavior Changes: Any new anxieties, aggression, lethargy, or restlessness?
  • New Lumps/Bumps: Note their location, size, and if they feel hard or soft.
  • Recent Diet Changes: Have you switched food, added new treats, or tried any “human foods”?
  • Recent Activities/Travel: Any exposure to new environments, other animals, or potential toxins?

Section 2: Questions to Ask Your Veterinarian

Don’t be afraid to speak up! Your vet wants to answer your concerns.

  • “Based on what you’ve found, what do you think is going on?”
  • “What are the next steps? (e.g., tests, treatments, follow-ups)”
  • “Are there any alternative treatment options we should consider?”
  • “What are the potential side effects of this medication/treatment?”
  • “What should I expect during recovery or over the next few days/weeks?”
  • “Are there any lifestyle changes I should make for my pet (diet, exercise)?”
  • “When should I schedule the next appointment, or when should I call if things change?”
  • “Can you explain the diagnosis/treatment plan in simpler terms?”
  • “What is the estimated cost of the recommended tests/treatments?”
